How to submit a completed statement

HOW TO SUBMIT A COMPLETED STATEMENT

This short video will show you how to submit a completed statement on the Department of Education Online Transition Learning and Development Statement.

The final submission section is available once you have completed the previous sections and marked sections 2 and 3 as either provided or absent.

On this page, we can now complete the final submission questions and assign the destination school.

Confirm if the child will be attending a school in Victoria.

Confirm if a completed copy of the statement is with the family.

You will need to confirm if the family has been made aware of to who and why the service will be sharing the transition statement.

You will need to check the tick box if the family opt out of sharing.

How the answers are set will decide if the Submit TLDS button at the bottom of the page becomes active.

If the family have opted out of sharing, the Submit TLDS button is disabled to prevent it being sent via Insight against the parents’ wishes.

As such, you can only download completed TLDS or mark it as completed but not sent via Insight.

In this situation, print 2 copies of the completed TLDS for the child's family, one for their own record and one should they later choose to share the TLDS with the child's school.

Then select Completed but not sent via Insight to complete that child's online TLDS.

For children attending government schools within Victoria, select Government from the sector selector and locate the correct school in the school's field.

Download and print a completed copy of the TLDS to provide to the child's family.

When you're ready and if the family have not opted out of sharing, select the Submit TLDS button.

For children attending Catholic or independent schools within Victoria, select Catholic or Independent from the sector selector and locate the correct school in the school field.

Download and print a completed copy of the TLDS to provide to the child's family and if the family have not opted out of sharing, select Submit TLDS button.

If the school's name does not appear in the drop-down list, select the School not listed tick box.

This will mean the TLDS will need to be submitted to the school manually.

You will need to print 2 copies of the TLDS to provide to the child's family in order for them to share a copy with the destination school.

Select Completed but not sent via Insight.

The statement will now be locked. If the destination school is unknown or outside of Victoria.

Print 2 copies for the child's family, one for them and one for them to deliver to the destination school.

You will not be able to submit TLDS because the TLDS cannot be sent via Insight to an Interstate or International School.

Select completed but not sent via Insight.

The statement will now be locked.

Remember to download or print copies to retain for your service records and you can also print an additional copy if the family intend to share a copy with the outside school hours care service.

Returning to the TLDS homepage, you can confirm that the statement is now marked completed and submitted.

If you need to edit the statement further, you can click the recall button, then it will be available for you to make alterations and resubmit.

If the destination school was not listed, or if the child is attending a school outside of Victoria, they will be marked completed but not submitted.
