A dedicated Specialist Housing Coordinator will help you find a home that suits your needs. Tell us where you want to live, your SDA funding category and anything else you think is important.
If the home has other residents living there, we will arrange a meet and greet so you can get to know each other and see if you would like to live together.
If you are happy with the home, we will assess your application and let you know the outcome.
Once you accept the offer, we will set up a time to formally sign you up to the property as an SDA resident. We will send you a Residential Tenancy Agreement pack that includes:
SDA Service Agreement – This agreement allows us to claim SDA funding from your NDIS plan. It needs to be signed by you or your plan nominee with commencement date of this agreement.
SDA residency agreement – This explains the rights and responsibilities of residents and SDA providers under Victorian rental laws.
SDA residency agreement information statement – This explains the Residency Agreement. We will provide you a copy of this document and explain it in a way that you can understand. This could be by talking to you and giving you something in writing.
Direct Debit Service Agreement and Direct Debit Request Form – This allows the SDA provider to collect rent from you. The direct debit request form needs to be signed by your financial administrator.
We need two weeks’ notice of your move-in date to get your Agreements (lease) and rent payments set up, before you move in.
You need to work with your Supported Independent Living (SIL) provider to set up your services with them. The staff working in the house are your SIL support.
