Information about how the department works collaboratively with SIL providers to deliver the best outcomes for residents.

Collaboration agreements

SDA providers and SIL providers are required to have a written agreement in place that clearly outlines each provider’s roles and responsibilities when supporting residents in SDA.

SIL onboarding

The department will arrange an onboarding meeting to help the provider understand the property, the residents and our roles and responsibilities under the Collaboration Agreement.

Notification of SIL service withdrawal

If a SIL provider plans to stop delivering services at a property, they must provide written notice to the department. Notice must be given before the end date or termination date of the Collaboration Agreement. The SIL provider must follow all responsibilities outlined in the Collaboration Agreement to support a smooth transition for residents.

Property exit and/or return

A property return process occurs if all residents leave a property. If this happens, one of the following will occur:

  1. Where the SIL provider continues to deliver services at other department-owned SDA properties, the property will be removed from the providers Collaboration Agreement Property List.
  2. Where the SIL provider is no longer delivering services at any department-owned SDA property, a letter will be provided to end the Collaboration Agreement.
