  • Letter of support from employer/host employer, teacher, trainer
  • Signed copy of the Nominee Declaration Form by nominee
  • Photo of nominee
  • Letter of support from employer/host employer
  • Signed copy of the Nominee Declaration Form by nominee
  • Photo of nominee
  • Letter of support from stakeholder
  • Full-colour logo

Partnership Award

(in addition to above)

  • Signed copy of the Partner Declaration Form
  • There is a limit of 10 pages of supporting evidence allowed per nomination.
